Required Documents

This is the list of required documents for applicants. You must submit your documents electronically as “Attachments” in our online application system. Before you can upload any of the required documents, you must create your online application account. Note: If you have previously completed and submitted an online application, you must reactivate your account before you can begin the new application process. Do not create a new user account. How to properly name and prepare your documents for submission

Before uploading any documents to our online application system, please name each file with your full name and the document name. For example, if your name is John Smith and you are preparing to upload a PDF of your transcript, please name the file: “John Smith Transcript.pdf”.  You may also submit a clearly legible scan saved as PDF, but you must name it correctly or it will not be accepted.

This consistent labeling system enables us to see clearly that you have uploaded all necessary documents.  Any documents not submitted in this format will not be reviewed.
